  • Optional Supplemental Insurance: While joining UBA does not require purchasing supplemental group insurance, these membership plans are available for members who wish to enhance their membership benefits. Members can choose from a variety of optional supplemental group insurance or non-insurance Benefit Boost membership plans based on their needs.

Because the UBA enrollment platform also includes non-insurance benefits and individual insurance that does not require UBA membership to enroll, the option to join UBA becomes your first step in the enrollment process. You can choose to not enroll in UBA and still enroll in these non-insurance Benefit Boost subscriptions or the VSP Individual Vision plan.

Once you've decided on your membership, you will need to enter your basic household information. This typically includes:

  • State of Residence: Enter the state where you currently reside. This information is crucial as plan availability and options can vary by state.

  • Obtain a Digital ID Card: A digital version of the ID card is available for immediate use before the physical card arrives.
  • Utilize Membership Guides: Comprehensive guides are available to help members understand how to maximize their benefits.

This letter will be mailed to you and serves as an introduction to your UBA membership. It will provide crucial information such as:

  • UBA Membership ID Number: Your unique identifier within the association.
  • Effective Date: The date your membership and any selected membership plans become active.
  • Plan Details: A list of all optional supplemental Membership or Benefit Boost subscription plans you have enrolled in, along with their respective membership plan ID numbers and the members enrolled on those plans.
  • Billing TPA Information: The name, phone number and email of the Billing Third-Party Administrator. In some cases, the welcome letter will also include the billing ID# and billing portal link. This is the information on who is drafting the monthly membership dues and would also be the contact for any cancellation or billing changes.

The welcome letter will also guide you on how to access various member resources through our Member Portal, including:

  • Member Guides: Information on how to make the most of your UBA Membership or Benefit Boost Subscription, available online for easy access.
  • Digital ID Cards: Instructions on downloading your digital ID card(s), which you can save on your phone, tablet, or print from the Member Portal.
  • State-Specific Certificates of Insurance: If applicable, you will have access to these documents, which outline the coverage details specific to your state.

Benefit Boost Subscription ID Cards

For those who have enrolled in Benefit Boost Subscriptions or the VSP Individual Vision Plan, please note:

  • Digital-Only ID Cards: All ID cards for the a la carte health and wellness Benefit Boost Subscriptions as well as the VSP Individual Vision Plan will be available exclusively as digital cards. This format helps keep the subscription costs low while providing valuable services. With relation to Healthcare2U Direct Primary Care Plans or the VSP Individual Health Plan, no ID card is necessary. For HC2U DPC plans, you simply call the PAL number listed in the member portal for service. For VSP, you simply need to instruct the provider that you have VSP and they can look you up to verify coverage.
  • Accessibility: You can download these digital ID cards to your mobile device or print them from the Member Portal for convenience.



By following the instructions in your welcome letter and utilizing the Member Portal, you can seamlessly access your membership benefits and ensure you are equipped with all necessary information and tools to optimize your UBA experience.
