Membership in the Healthy America Association or the United Business Association (depending on plan choice) is required to purchase our GAP Plans. Your effective date is listed on your welcome letter with your member guide.

There are no Critical Illness or Accident benefits in the GAP Basic Plan^SM. The Accident benefit is not available with the GAP CI Plan^SM.

For the GAP Plus Plan^SM and the GAP 10K Plus Plan^SM, the Accident and Critical Illness benefit is not available in CT, MA, NH, NY and VT. Accident benefit reduces by 50% at age 70. Crticial Illness benefit ends at age 64. There is a 30 day waiting period after the effective date for the Accident benefit. There is a 12 month waiting period after the effective date for the full benefit schedule of the Critical Illness benefit (if first-diagnosis occurs in the first 12 months, a reduced benefit will be paid.) The Critical Illness benefit is for primary and spouse only - not dependent children.

For the GAP CI Plan^SM, the Critical Illness benefit is not available in CT, MA, NH, NY and VT. Critical Illness benefit ends at age 64. Medical Travel Assistance Plan Benefit for all GAP Plans is not available to Connecticut or Florida Residents.

No GAP Plan is available in North Carolina.
